Mandatory Vendor Partner Profile Requirements

• Provide 24/7 hour emergency service within 4 hours

• Billing submitted within 15 days

• All necessary licenses and insurance coverage, including General Liability ($1 million dollars per occurrence, $2 million dollars aggregate) and Workers Compensation (regardless of state requirements).

The Vendor Partner application is intended for the sole use of new vendors to TSFM. If you are currently providing services for TSFM please DO NOT submit an application. If you provide services across multiple disciplines, please reflect your primary business line and fill in other service offerings in the “additional services” section of the application.
